If you would like to know more about getting help at home with Mercy Community, through one of our home care packages, there are five simple steps to follow.
Step 1. Call Our Team
To access one of Mercy Community's home care packages, call our Home Care Coordinator on 0488 403 330. Our friendly staff will have an initial conversation with you to get an understanding of your situation and explain the home care process.
Step 2. Check your Eligibility for the Subsidy
Some Home Care services are distributed by the Australian Government for individuals. You can check your eligibility for a package by visiting the My Aged Care website or simply talk to our Home Care Coordinator about it and they can guide you through the steps.
Step 3. Meet and Plan
If you are deemed eligible for one of our home care packages, we will arrange for one of our friendly Community Coordinators to come and visit you. During this meeting you can discuss your needs with the Coordinator and create a service plan for you.
Step 4. Finalise your Service Plan
Your Mercy Community Coordinator will finalise your service plan and funding if applicable.
Step 5. Begin Services
Your Home Care services can begin the same week that steps 1 to 4 are finalised.
