Residents who have been offered a place at one of Mercy Community Services (MCS) facilities are invited to participate in the transition to care program, which operates prior to and following admission.
The objectives of the program include:
- creating a relationship between the resident, their family and the service to support the transition experience
- communicating the philosophy of care, services and support available for the resident and their family members
- determining resident special needs and preferences and discussing how these could be met – maximising the opportunities for person centred care
- answering practical questions and advising what to expect in the first few weeks of occupancy.
